This machine converts the electricity from 220 down to 110 making it safe to charge/use a US device such as a cell phone or radio. An adapter only allows you to plug the device into the outlet and does not reduce voltage. If your device doesn't have the ability to convert the power internally then it will be ruined.
The converter is rated for 200w maximum. I had to do a little Googling on hair straighteners, but it appears their rated wattage is generally inaccurate or an average, and actual wattage jumps to around the 200w mark as the thermostat cycles on and off. It might work, but you'd be pushing it. When I searched for straighteners on Amazon, it appeared most of them were rated for dual voltage--you may want to double check the one you own or the company's website as it may also operate at 220v, in which case all you'll need is a plug adapter and can skip the converter altogether.
Yes it will work in Italy. I was there in March of this year and it worked perfectly in Venice, Florence and Rome.
